Bichectomy is a comfortable 30–45 minute procedure generally performed under local anesthesia, completed while conversing with the patient.
A tiny 1 cm incision, invisible from the outside, is opened from the inside of the cheek.
Bichat’s fat pads are reached. Not all of these fats, but a portion sufficient to thin the face (40–50%), is gently removed.
The incisions are closed with 1–2 self-dissolving sutures.
Immediately after the procedure, the patient can return home or to work. There are no visible scars, sutures, or bandages on the cheeks from the outside.

Despite the fat removal, the cheeks may swell during the first 1–2 weeks due to surgical trauma (as if having a tooth extracted). During this process, the face may look more swollen rather than thinner; this is completely normal.
For the first 3 days, hot, hard, and grainy foods should be avoided; mouthwash should be used after every meal.
The subsiding of edema and the emergence of cheekbones as the cheeks curve inward begins at the 3rd week, while the full result settles by the 3rd month.
This is the biggest concern regarding bichectomy. Bichat fat is different from superficial cheek fat and does not melt with age. However, Dr Bekir Can Gümüşlü does not remove all of this fat, but only the excess (preservative surgery), thereby eliminating the risk of future sagging.

Nothing should be eaten for the first 2-3 hours after surgery until the numbness wears off. After that, it is sufficient to eat foods such as warm soup and purées on the first day. Normal eating can be resumed the following day.
