As the name suggests, closed rhinoplasty is a method where all procedures are performed entirely through the nostrils without any external incisions. This technique is considered the gold standard for patients with high aesthetic concerns, as it preserves the ligaments and vascular structures supporting the nasal tip while leaving no visible external scars.
The most common point of confusion for our patients is the difference between these two methods. In the open technique, the column of skin between the two nostrils, called the “columella,” is cut to lift the nasal skin completely. In the closed rhinoplasty method, the columella is not cut. All incisions remain within the mucosa inside the nostrils.
This difference is not just about “scarring.” Because the vessels supplying the nose and the lymphatic drainage channels are not severed, blood circulation is not disrupted. Op. In the past, there was a misconception that the closed technique could only be applied to simple cases. However, with today’s advanced surgical instruments and techniques, aquiline noses, drooping nasal tips, wide noses, and even noses with breathing problems (deviation) can be successfully operated on using the closed method.

The process following surgeries performed with the closed technique is quite comfortable. The silicone splints and the external cast placed at the end of the surgery are removed after 1 week. Since the sutures are inside the nose, they dissolve on their own, eliminating the stress of suture removal.
Bruising is minimal thanks to the tissue-preserving approach, though it can vary depending on the patient’s skin type. When the casts are removed at the end of the first week, our patients typically enjoy their new look immediately, even with minor swelling.
From the surgeon’s perspective, it requires greater technical skill and experience due to the limited field of view. Whether the technique is open or closed is not a factor determining nasal tip drooping. Nasal tip drooping is related to insufficient supporting cartilage. In the closed technique, nasal tip drooping is not expected because the internal nasal ligaments are preserved and supported by structural grafts.
Absolutely. During closed rhinoplasty, problems such as septal deviation (bony curvature) and turbinate (nasal mucosa) enlargement are also corrected during the same session, through the same incisions. Both aesthetic and functional improvement is achieved.
In the closed technique, all stitches are inside the nostrils and absorbable sutures are used. Therefore, there is no need to have the stitches removed; they dissolve on their own over time.
