While classic facelift surgeries primarily address sagging in the lower half of the face and the neck, a different approach is required for volume loss in the middle region of the face (cheeks and eye area).
Endoscopic Facelift, also known as Midface Lift, is a high-tech surgery performed with the help of high-resolution cameras (endoscopes) through millimetric holes entered from within the scalp, without making large incisions. The greatest advantage of this method is that it leaves no visible scars on the face and offers a result that looks “volumized” rather than “pulled.”
On a young face, the malar fat pads are positioned over the cheekbones. With aging and gravity, these fat pads slide downward toward the sides of the nose. Consequently:
Hollows and bagging form under the eyes.
The cheekbones flatten.
The lines at the side of the nose (nasolabial folds) deepen.
The face takes on a rectangular and tired expression.
The endoscopic facelift steps in at exactly this point. It moves these downward-slid tissues vertically upward, opposite to the direction of gravity, and fixes them back where they belong.
The surgery is performed under general anesthesia.
Small incisions of approximately 1-2 cm are made within the hair in the temple area.
Thanks to endoscopic cameras, the surgeon advances safely by viewing the vascular and nerve structures beneath the skin on a large screen.
The soft tissues and fat pads separated from over the cheekbone are pulled upward as a single unit.
The repositioned tissues are suspended to the bone structure or the strong temporal fascia with sutures.

Sensation: There may be temporary numbness in the treated areas, which improves over time.
Scar: Since the scars remain within the hair, the patient does not have to worry about scarring after surgery.
Longevity: Because deep tissues are moved rather than just the skin, the results are quite long-lasting.
An endoscopic midface lift gives the patient back “the facial features they had when they were younger.” While a classic facelift “irons” the face, an endoscopic facelift provides “volume and form.”
It is ideal for people aged 30-50 who do not have excessive skin or sagging but whose cheeks have drooped, whose under-eyes have hollowed out and who appear tired. If there is excessive skin sagging, it should be combined with a classic facelift.
During endoscopic mid-face lift, the temple area is also lifted, causing the corners of the eyes to rise slightly. This creates a more dynamic and attractive look (Fox Eyes / Bella Eyes). If the patient does not want this, the surgical plan is adjusted accordingly.
The incision heals more quickly, but the swelling in the face may take slightly longer to subside compared to a classic facelift. It may take 1-2 months for the tissues to settle into their new position.
As the incisions are made within the scalp, temporary localised hair loss (shock loss) may rarely occur in that area. However, permanent hair loss may occur in very rare cases.
Depending on the care given, it lasts between 7 and 10 years.
